I''m 12 weeks and I''ve just had vaginal bleeding that had soaked through my clothes, but no pain. What could this mean?

The first thing to do is for you to get checked out by a doctor straight away, especially if you are still bleeding. Usually if you are miscarrying, it is associated with severe abdominal cramps but sometimes there is bleeding before the pain. So your doctor will probably arrange an ultrasound scan to establish whether the baby is still alive, or whether you are having a miscarriage. Remember that it is still possible for the bleeding to be from the developing placenta, and for the baby to be unaffected by this.
