The midwife will tell you. She will listen for changes in your breathing as you start to grunt, resisting the urge to push. An internal examination will confirm that your cervix is fully dilated - the marker for the start of second stage. Or, if progress is very rapid, you may start to feel and see your baby''s head emerging.
